Maleck Therapy is currently in network with the following insurance providers:

  • Dean

  • Quartz

  • Trilogy Health Plan

  • Dane County CCS

  • Wisconsin Medicaid/Badgercare (Including I-Care and My Choice Wisconsin Health Plan)

  • The Alliance

Therapy services may be covered in full or in part by your insurance company. Please check your coverage carefully by calling your insurance provider prior to setting up your initial appointment. When calling your insurance company to verify your benefits, the following questions may be helpful:

  • Does my plan include mental health benefits?

  • What is my deductible and has it been met?

  • Is there a limit to the number of sessions my health insurance will cover?

  • How much is my co-pay?

  • Is approval required from my primary care physician prior to receiving therapy?

Rates

If we are not in network with your insurance, or if you choose not to use your insurance, you may choose to pay for your session(s) out of pocket. The rate that these sessions are billed at will be discussed with your prior to your initial appointment and vary based on the level of training of your therapist. We have therapists available at three different price points to help you find a provider that will work for you and your financial situation.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of their expected charges for medical services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
